Payment and insurance
Currently, we can bill most United Healthcare, Aetna, and Cigna PPO plans, with no co-pay from you.
If you have United or Aetna insurance, send a picture of your insurance card, along with your birthdate and full name to (480) 242 5588 so we can verify your coverage.
If you have Cigna insurance, click the button below to verify your coverage.
If your insurance does not cover your visits, or if you do not have insurance, you can pay out of pocket for your visit. You may use HSA/FSA funds for this purpose.
If you are part of a Health Share we can provide you with an itemized receipt for reimbursement.
After you have booked a visit, you will receive a confirmation email with access to your intake forms.
Out of pocket cost of visits varies depending on time spent.
Most visits are $120 to $220 (click the booking link to see exact cost)
After your visits, you will have access to ongoing communication with your lactation consultant in our app
